Medical Plan

Upon Hire

1. CIGNA PPO Plan
2. Large provider network including Mountainside Hospital IPA.

Dental Plan

1st of the month following 1 year of service for employees regularly scheduled to work 36 hours or more weekly.

2. Two coverage options

Prescriptions

Upon Hire    

1. Large provider network
2. Mail order prescription service.

Vision Plan

Upon Hire       

1. Included in the CIGNA PPO medical plan.
2. Coverage for exams and discounts on glasses and contacts.

Life Insurance

Upon Hire

1. Two times employee’s annual salary.
2. 100% paid by Mountainside Hospital.

Supplemental Life Insurance

1st of the month following 3 months of service for employees regularly scheduled to work 36 hours or more weekly.

1. Employees may elect coverage up to five times their annual salary.
2. 100% paid by employee

Short Term Disability

Upon Hire

1. New Jersey State Disability Program. 
2. Automatic enrollment for employees.

Paid Time Off (PTO)

1. 23 days

Flexible Spending Accounts (FSA) –  Health Care

1st of the month following 3 months of service for employees regularly scheduled to work 22.5 hours or more weekly.

1. Tax free dollars used to pay for expenses not covered under the medical plan.
2. Current CoreSource participants may continue under the existing plan in 2007. New FSA plan to be introduced in 2008.
