| How It Works |
The HomMed Health Monitoring System is a simple-to-use health monitor that collects a patient’s clinical vital signs on a regular basis. Using voice prompts and visual cues, the monitor guides the patient through the vital signs process, measuring weight, temperature, blood pressure, blood oxygen saturation, and pulse. To get a more comprehensive picture of the patient’s health, the monitor asks up to ten health-related questions. Multiple peripheral attachments are available, including a glucose meter, spirometer, PT/INR System, ECG unit, and more. All data is immediately transmitted to our agency for a clinical review. If the data collected indicates a need for intervention or if the user fails to collect their vital signs when they are prompted, appropriate action is taken and members of the care team may be contacted for intervention.

| Benefits |
The HomMed Health Monitoring System:
- Provides a sense of security for the patient and patient’s family knowing that somebody is watching over his or her health on a regular basis.
- Provides care based on patient needs, not only as a nurse’s schedule allows.
- Lets the doctor adjust medications or diet based on trend over time data.
- Catches irregularities early, before they become problems, which reduces emergent care episodes, which keeps the patient out of the ER and in the physician office.
- Acts as an educational tool to reinforce medication use and diet restrictions.
The HomMed System is beneficial for almost anybody, especially those who have heart disease, lung disease, high blood pressure, renal failure, advanced diabetes or other diseases where small changes in condition have large health implications.
The HomMed Health Monitoring System is an FDA Class II Medical Device.
