Home Care Service
Whether you're recovering from a knee replacement, coping with diabetes, or improving function impaired by a stroke, Mountainside is there.
We're there to provide medical and emotional support and rehabilitation in the comfort of your own home. With more than 116 years of experience, we serve Essex county with our onsite home care nurse liasons, providing medical and emotional support when needed, as well as assistance while recovering from an injury or illness.
Your doctor may prescribe home care to:
- Prevent or postpone hospitalization or nursing home care
- Aid in recovery from surgery after discharge from the hospital
- Provide follow-up to a hospital stay
Once your doctor orders home care, we verify your insurance benefits and eligibility so you don't have to worry about it. Please note that we accept most medical insurance plans, including Medicare and Medicaid.

Rest assured that you'll be in good hands. Your doctor and a registered nurse will work together to develop a plan of care that's just right for you.
- Disease Management — coordination of care and education for conditions such as heart disease, stroke, cancer and diabetes, directed by clinical nurse specialists.
- Rehabilitation for Total Joint Replacement & Other Orthopedic Conditions — physical and occupational therapy.
- Stroke Rehabilitation — physical, occupational and speech therapy.
- Behavioral Health Services — short-term counseling and medication monitoring for those with mental health disorders.
- Wound, Ostomy & Continence Care — education and care of surgical or pressure wounds and instruction for new colostomy patients, directed by clinical nurse specialists.
- Clinical Nurse Specialists — available to aid in your treatment of wound, ostomy, cardiac or diabetes care.
- Skilled Nursing Care — regularly scheduled visits by a registered nurse who attends to medical needs and coordinates care provided by other professionals.
- Home Health Aide Care — personal care (bathing, meal preparation, etc.) by a certified professional. Private duty care is also available at home, nursing facility or in the hospital.
- Tele-Health Monitoring System — a simple-to-use health monitor that collects a person's vital signs on a regular basis, then transmits that data to our agency for clinical review. For more information about home care tele-monitoring, click here.
- Therapeutic Counseling & Community Resource Referrals — assessment, counseling and referrals by social workers help patients cope with illness (financially, emotionally, etc.).
- Nutritional Counseling — education on proper nutrition by registered dieticians.
- Medical Supplies & Equipment — arrangement for delivery, maintenance and pickup of necessary items (hospital beds, wheelchairs, oxygen equipment, etc.).
